CMS Releases Updated MDS 3.0 Quality Measures User's Manual

The Centers for Medicare and Medicaid Services (CMS) published a new MDS 3.0 Quality Measures user manual version 18.0, effective January 1, 2026. This manual provided updated specifications for all MDS-based quality measures, including risk-adjustment data used in public reporting and quality comparison. The manual includes a “notable changes” section that summarizes updates from the prior version, helping providers understand what elements in quality measurement have shifted for 2026. 

New data validation activities starting in 2026
Beginning January 2026, CMS initiated targeted data validation reviews of MDS submissions. Selected SNFs are randomly chosen to have portions of their MDS data validated for accuracy as part of the SNF Value-based Purchasing (VBP) and Quality Reporting Program (QRP) processes. Notices go to the providers and selection letters are uploaded in the CMS iQIES system. This step emphasizes CMS’s commitment to ensuring that MDS data used in quality calculations are reliable and clinically supported.
